New Patients

Psychiatry — Dr. Lazarevic — Initial psychiatric evaluations begin at 60 minutes. For complex histories, the assessment may extend across two to three appointments — it is often across those early sessions that the full treatment approach becomes clear. Intake paperwork and instructions will arrive by email after scheduling.

Dr. Lazarevic offers telepsychiatry for patients in New York, Connecticut, and California. Patients in other states are welcome to inquire.

Psychotherapy — Dr. Cohen — Initial psychotherapy appointments are 60 minutes. Dr. Cohen offers trauma-informed therapy, Eye Movement Desensitization and Reprocessing (EMDR), Cognitive-Behavioral Therapy (CBT), and psychodynamic approaches.

Nutrition — Helaine Polanco, MS — Nutritional consultations are available both a la carte and through Nutrition & Momentum Care™ Packages. Helaine offers inperson and telehealth appointments; patients are welcome to inquire about availability.

  • You will receive a confirmation call, in this call you will receive an orientation to the portal which you will set up to complete intake documents appointment type. Then your appointment will be confirmed.

    Please complete the intake forms before your first visit — they allow the care team to prepare an informed evaluation.

  • Yes. Initial appointments can be scheduled directly with the psychiatrist, the therapist or nutritionist based on your needs.

    If you are unsure which provider to start with, call the office at (212) 924-2871 — we will help you identify the right fit.

  • Please contact the office as early as possible if you need to cancel or reschedule, the policy is by noon the prior business day (not including major holidays). Late cancellations and no-shows are subject to a fee.

    Cancellations can be made via the front desk at (212) 924-2871 or through the secure portal system.

  • Yes. The patient portal is secure and because of this it is the primary way we share and recieve your documents.

    Evolution Medicine is a self-pay practice — no insurance claims are filed, and your care does not automatically appear on insurance records. All consultations and communications are confidential.
