Step 2: To Schedule Your Initial Consultation

Complete your registration and intake forms, then choose the right initial consultation for your needs:

  1. Psychiatry with Dr. Lazarevic : 60 minute Initial Psychiatric Consultation

    Best for: Depression, anxiety, ADHD, PTSD, bipolar disorder, addiction

  1. Psychology with Dr. Cohen : 60 minute Initial Assessment

    Best for: Psychodynamic psychotherapy, Trauma-focused therapy, EMDR, interpresonal issues, emotion regulation, addiction

  1. Nutrition with Ms. Polanco : 60 minute Nutritional Assessment

    Best for: Functional nutrition support for mental health and gut-brain optimization

Step 3: Complete Intake Forms

Before scheduling, complete intake paperwork through the portal.

  • Intake, release of information, treatment agreement, readiness assessment
  • Optional: Medical history questionnaire, nutritional and GI health forms, screening forms, health tracking forms
  • Upload labs from prior year
  • Upload insurance card (for prior auths and superbill purposes)

What to Expect at Your First Visit

Before Your Appointment:

✓ Arrive 10 minutes early to check in
✓ Bring current medication list (including all supplements)
✓ Bring recent lab work if available
✓ Bring insurance card (even though we're self-pay, for superbill)

During Your Appointment:

  • We discuss your complete history and current concerns
  • We review medical and psychiatric background
  • We order comprehensive lab work (if appropriate)
  • We create an initial treatment plan
  • You receive prescriptions if appropriate

After Your Appointment:

  • You receive lab requisition forms (most covered by insurance)
  • We schedule your follow-up appointment
  • You receive a detailed visit summary through the portal
  • You can message your provider with questions
