Evolution Medicine practices integrative psychiatry — combining conventional psychiatric excellence with functional and regenerative medicine to investigate and treat the root causes of complex mental health conditions. Three coordinated pathways provide access to this approach, each calibrated to a different level of clinical need.

Psychiatry & Psychotherapy

For adults navigating depression, anxiety, Attention-Deficit/Hyperactivity Disorder (ADHD), Post-Traumatic Stress Disorder (PTSD), bipolar disorder, addiction, trauma, and related conditions. This pathway provides access to psychiatric evaluation, psychotherapy, and functional nutrition — individually or in combination.

Psychiatric Care — Dr. Lazarevic holds board certification in General Psychiatry and Addiction Psychiatry, with advanced training in integrative, functional, and regenerative medicine. Evaluations are comprehensive and root-cause oriented, with attention to the biological, psychological, and behavioral factors driving symptoms. Medication optimization, functional testing, and coordination with other providers are available where appropriate.

Psychotherapy — Dr. Shuki Cohen, PhD specializes in trauma-informed approaches including Eye Movement Desensitization and Reprocessing (EMDR), Cognitive-Behavioral Therapy (CBT), Rational-Emotive Behavioral Therapy (REBT), psychodynamic therapy, and motivational interviewing.

Nutrition — Helaine Polanco, MS specializes in gut-brain health and functional nutrition, available a la carte or within Momentum Care™ Packages.

Addiction is woven throughout this pathway — not siloed as a separate service. Dr. Lazarevic's board certification in both General Psychiatry and Addiction Psychiatry makes Evolution Medicine one of the few practices equipped to address substance use and co-occurring mental health conditions with equal clinical depth.

Momentum Care™ Packages

For treatment-resistant and complex cases. When conventional care hasn't produced lasting results — or when multiple conditions require coordinated attention — Momentum Care™ Packages bring the full clinical team together in a structured, fixed-fee program over 6, 12, or 18 months.

Packages incorporate Dr. Lazarevic, Dr. Cohen, and Helaine Polanco working from a unified framework, with advanced functional and genetic testing where indicated. This is the appropriate pathway for patients who have tried multiple approaches without resolution, and for dual diagnosis presentations where addiction and underlying mental health conditions need to be addressed together.

Physician Wellness Program

For physicians, physician assistants (PAs), and prescribers under monitoring by CPH, OPMC, or HAVEN. Dr. Lazarevic provides Independent Medical Evaluations (IMEs), quarterly monitoring support, confidential medication optimization, and psychotherapy designed for the pressures of monitored medical practice. Over a decade of experience in this area. All care is self-pay; no insurance claims are filed.

  • Individual services are appropriate when you're beginning evaluation, working on one primary concern, or adding a specific provider to an existing plan. Momentum Care™ Packages are better suited for treatment-resistant or complex cases — when multiple conditions interact, when prior treatment hasn't worked, or when you're looking for a fully coordinated team approach from the start. If you're unsure, a consultation with Dr. Lazarevic will clarify the right fit.

  • Yes. Dr. Lazarevic, Dr. Cohen, and Helaine Polanco can work independently or as a coordinated team. Within Momentum Care™ Packages, team coordination is structured and built into the program. Outside of packages, providers can be engaged individually and may coordinate informally based on your treatment plan.

  • Yes. Many patients work with Evolution Medicine alongside other providers. Dr. Lazarevic can provide an integrative psychiatric evaluation, functional testing, or medication optimization as a complement to existing care. Please let us know during your consultation what other treatment is in place.

  • Yes. Dr. Lazarevic offers telepsychiatry for patients in New York (NY), Connecticut (CT), and California (CA). Dr. Cohen and Helaine Polanco also offer virtual sessions. Patients in other states are welcome to inquire.

  • Evolution Medicine is a self-pay practice — we do not bill insurance directly. Detailed superbills are provided after each visit, which you may submit to your insurer for potential out-of-network reimbursement depending on your plan. For patients navigating addiction, professional licensing concerns, or sensitive mental health history, self-pay also means no insurance claims are filed — care remains entirely private and does not appear on insurance records.
