How do I schedule an appointment?

What should I bring to my first appointment?
Please bring a current medication list (including supplements), any recent lab work, relevant medical records, and a list of previous treatments and what did or did not help. Complete intake paperwork through the patient portal at least 48 hours before your appointment.

Do you offer in-person and telemedicine appointments?
Both are available. Telepsychiatry is offered to patients in New York, Connecticut, and California. Patients in other states are welcome to inquire. Our office can help determine the right format when you schedule.

How much does treatment cost?
Psychiatry (Dr. Lazarevic): $750 · Psychology: $350 · Nutrition (Ms. Polanco): $295
All initial consultations are 60 minutes. Complex evaluations may require two to three appointments.

Do you accept insurance?
No. Evolution Medicine is a self-pay practice. We do not bill insurance directly. Self-pay means no restrictions on testing or treatment, no 15-minute appointments, and no insurance documentation of your psychiatric care. We provide detailed superbills for out-of-network reimbursement submission.

What forms of payment do you accept?
Credit and debit cards · HSA/FSA cards · Zelle · Wire transfer (for package programs). Payment is due at time of service. We do not accept personal checks.

How do I get reimbursed by my insurance?
Pay at time of service, submit your superbill to your insurer, and reimbursement is sent directly to you. Many patients receive partial reimbursement — the amount varies by plan. We recommend calling your insurer before your first appointment to ask about out-of-network outpatient psychiatric benefits.

Can I see multiple providers?
Yes. For complex conditions, coordinated care across psychiatry, psychology, and nutrition is available — and often recommended. Your team communicates regularly to align on your treatment.

Physician Services

What is an Independent Medical Evaluation (IME)?
A comprehensive psychiatric evaluation for physicians under professional monitoring (CPH, OPMC, HAVEN). Includes a 90–120 minute interview, collateral contacts, full record review, and a detailed clinical report.
Fee: $3,700. Payment plans available. Report released upon full payment.

Communication

How do I reach you with questions?
Call (212) 924-2871 — Monday through Thursday, 9:00 AM – 5:00 PM. We do not use standard email for clinical communication due to HIPAA requirements. Messages are monitored regularly and returned promptly on weekdays.

Additional Services

Do you offer group therapy?
Educational group programs are currently in development — covering nutritional psychiatry, lifestyle medicine, mindfulness, and functional medicine basics. Not yet available. Ask your provider or check the patient portal for updates.
