What should I bring to my first appointment?
Please bring:
- Current medication list (including all supplements and over-the-counter medications)
- Recent lab work (if available)
- Medical records (if you have complex medical history)
- Insurance card (for superbill purposes, even though we're self-pay)
- List of previous treatments and what did/didn't work
Complete intake paperwork through the patient portal at least 48 hours before your appointment.
Do you offer telemedicine appointments?
Initial consultations are conducted in-person for the most comprehensive assessment.
Follow-up appointments may be conducted via telemedicine when clinically appropriate.
Pricing & Insurance
How much does treatment cost?
Initial Consultations:
- Psychiatry (Dr. Lazarevic): $750 (60 minutes)
- Psychology (Dr. Cohen): $350 (60 minutes)
- Nutrition (Ms. Polanco): $250 (60 minutes)
For complex presentations, a thorough psychiatric evaluation may require 2-4 visits.
Follow-up visit pricing varies based on frequency and complexity. We'll discuss this during your initial consultation.

Do you accept insurance?
No, Evolution Medicine is a self-pay practice. We do not accept insurance or bill insurance directly.
Why self-pay?
- Spend adequate time with you (no 15-minute appointment restrictions)
- Order comprehensive functional testing without insurance denials
- Focus on your clinical needs, not insurance company guidelines
- Protect your privacy (no insurance documentation of psychiatric treatment)
We provide: Detailed superbills you can submit to your insurance for out-of-network reimbursement.
Typical reimbursement: 50-80% for out-of-network psychiatric care (varies by plan)
What forms of payment do you accept?
- Credit and debit cards
- HSA/FSA cards
- Zelle
- Wire transfer (for package programs)
Payment is due at time of service. We do not accept personal checks.
How do I get reimbursed by my insurance?
- Pay for your appointment at time of service
- We provide a detailed superbill (itemized receipt with diagnostic and procedure codes)
- You submit the superbill to your insurance company
- Insurance reimburses you directly based on your out-of-network benefits
Check your benefits: Contact your insurance and ask about "out-of-network outpatient psychiatric care" reimbursement percentage.

Who will I see for treatment?
That depends on your needs:
For general psychiatry and medication management: Dr. Lazarevic
For addiction treatment: Dr. Lazarevic (board-certified addiction psychiatrist)
For psychotherapy: Dr. Cohen or Dr. Lazarevic
For nutritional support: Ms. Polanco
For complex, treatment-resistant conditions: Coordinated team approach (all three providers)
Can I see multiple providers?
Yes. For complex conditions, we often recommend coordinated care:
Example treatment team:
- Dr. Lazarevic for medication management and functional medicine
- Dr. Cohen for treatment resistant, complex or trauma-focused psychotherapy
- Ms. Polanco for nutritional optimization
Your team meets regularly to discuss your progress and coordinate treatment.
Physician Services
What is an Independent Medical Evaluation (IME)?
An IME is a comprehensive psychiatric evaluation requested by monitoring entities (CPH, OPMC, HAVEN) for physicians under professional monitoring.
Dr. Lazarevic's IMEs include:
- 90-120 minute comprehensive interview
- Collateral contacts (family, colleagues, previous providers)
- Thorough record review
- Detailed clinical report with recommendations
Fee: $3,600 (includes all interviews, collateral work, and report)
Payment plans available. Report released upon full payment.

Why not email? We don't use standard email for clinical communication due to HIPAA security requirements and the need for nuanced communication that email doesn't permit.
Rest assured: Phone messages are monitored regularly and returned promptly on weekdays.
